ABSTRACT

Objective: To observe the clinical efficacy of Hegan granule in the treatment of non-erosive reflux disease with liver stagnation and spleen deficiency syndrome and its effect on quality of life, 5-hydroxytryptamine (5-HT) and vasoactive intestinal peptide (VIP) level. Method: Patients with non-erosive reflux disease of liver stagnation and spleen deficiency were randomly divided into treatment group (32 cases) and control group (33 cases). The treatment group was orally given Hegan granules after meal, 10 g/bag, 1 bag/time, 2 times/day. The control group was orally given with omeprazole enteric-coated tablets twice daily, 20 mg/grain. Both groups were treated for 8 weeks. The total scores of traditional Chinese medicine (TCM) symptoms, SF-36 health scale scores, 5-HT and VIP levels were observed before and after treatment. Result: The medical efficacy of the two groups was 87.5%and 81.8%in the treatment group and the control group, respectively. The treatment group was superior to control group (PPPPConclusion: Hegan granule has a good clinical efficacy in treating patients with non-erosive reflux disease and liver stagnation and spleen deficiency. It can significantly alleviate the symptoms of TCM, improve the quality of life of patients, and reduce the sensitivity of esophageal viscera.

ABSTRACT

Objective To study the airway patency during sitting position and supine position in bronchoscopy during anesthesia. Method Forty-eight patients underwent bronchoscopy during Non-intubation anesthesia were randomly allocated to two groups.There were 24 cases in each group by observeing the airway patency and vital signs of the two groups during the procedure. Result Sitting position in bronchoscopy during anesthesia have the shorter average time to reach the carina, on securing a clear airway. The incidence of desaturation, hypertension and agitation were lower in sitting position during bronchoscopy. Conclusion Bronchoscopy anesthesia can reduce discomfort and fear, but traditional supine bronchoscopy anesthesia can cause displacement of the tongue, collapse of the upper airway in the oropharynx, soft palate and vocal cord related to gravity may seriously obstruct the upper airway, not only brings inconvenience to the inspection, but also increases the risk of airway correlation and hemodynamic instability, Sitting bronchoscopy ensure the airway patency and maintain a more stable hemodynamic state, so that sitting position in the sedation of bronchoscopy anesthesia is safer and more convenient.

ABSTRACT

This study was aimed to investigate the sensitive index for predicting the sensitivity of patients with multiple myeloma (MM) to glucocorticoid (GC). MM1.S, MM1.R cell lines and 10 MM patients sensitive or non-sensitive to GC were enrolled in this study. The expression of heat shock protein 90 (HSP90) and P23 in peripheral blood mononuclear cells (PBMNC) of MM patients was detected by Western blot, and its relation with GC resistance was explored. The results showed that there were no differences in HSP90, GR and IκB-α expressions among MM1.S, MM1.R, PBMNC of normal persons and MM patients, while there was apparent difference in P23; along with increasing of dexamethasone dose, the survival rate of MM1.S decreased, but that of MM1.R did not change so obviously. Among 7 cases with P23 low expression, combination treatment scheme with GC was effective in 6 cases (86%), uneffective in 1 cases (14%); while among 3 cases with P23 high expression, 1 cases (33%) showed effective to combination treatment with GC, 2 cases (67%) showed uneffective (P < 0.05). It is concluded that the P23 expression may predict the sensitivity of MM patients to GC and may be used as index for guiding clinical treatment.
